“The instrumental of Armanisem Photography owned and operated solo company
that Also accepts contribution to direct charitable posts.”

• All photos and digital contents are the property of photographer (Arman
Khosrowbeyk) and cannot be reproduced or sold unless given exclusive
permission from the photographer. If permission is granted, the person or persons
understand that he/she must give or acknowledge proper credit of who is the
original photographer aka (Ar.k)
• The Armanisem photography (Ar.k) will not be held responsible for any damage,
liability or injury (“personal”) caused while photos are being taken or in any
location nor from the publishing of any photos taken by AR.K Armanisem
Photography that are placed anywhere online (websites, personal pages,
advertisements, falsifying image places or other social media sites such as
Instagram, Snapchat, Facebook, Twitter, Myspace and etc.. as well as on other
social media websites).

Authorization

I ___________________________ hereby authorize AR.K Armanisem
Photography to publish photographs taken of me as well as my name and
likeness for use in print, online and video-based marketing materials and other
company publications from any photo session for:

I agreed to participate in/or have asked the photographer to do Photography of

I hereby sign authorization of release and hold harmless any accusations against
AR.K Armanisem Photography from any reasonable expectation of privacy or
confidentiality associated with the images specified above.
I further acknowledge that my participation is within and that I will not receive
financial compensation of any type associated with the taking or publication of
these photographs or participation in company marketing materials or other
Company publications beside what we agreed before. I acknowledge and agree
that publication of said photos confers no rights of ownership whatsoever.
I hereby for releases this agreement to AR.K Armanisem Photography, contractor,
employees, and any authorized third parties involved in the creation or publication
of Photography material, from liability for any claims by me or any third party in
connections with my participation.

Armanisem Photography MODELING Liability and Release Form

This Document Affects Your Legal Rights, Read It Carefully

I. LIABILITY RELEASE
I,______________________, having agreed to act / model while participating in
the following photo shoot with Ar.K Armanisem Photography on the dates of
,__/__/_____ and knowing that there are certain risks and dangers related to this
Activity that I hereby agree with:
(1) I am aware of all the risks and dangers associated with participation in any
transportation to and from any places that photo shoot done with Ar.K Armanisem
Photography.
(2) I agree that Ar.K Armanisem Photography, its contractors, professional
photography and any other sponsors/providers who is associated with and their
employees shall not be legally responsible for any loss, injury or damage resulting
from any cause, including negligence of any third party associated.
(3) I understand and agree that any equipment, outlet, or decorative objects that I
am using while modeling I used at my own risk. I understand and agree that Ar.K
Armanisem Photography shall not be liable for any loss, damage or injury
resulting from the use of equipment and Ar.K Armanisem Photography and any
manufacturers make no warranties regarding use or suitability of the equipment.
(4) I agree to take all risks in connection with any photo or video session with Ar.K
Armanisem Photography including, but not limited to, risks associated with the
location and any actions I may take while modeling. To the fullest extent allowed
by law I agree to RELEASE, HOLD HARMLESS and INDEMNIFY Ar.K
Armanisem Photography and their employees, agents or contractors from any
liability for death, loss, injury or other damage which may occur/appear during or
as a result of any photo or video session done through Ar.K Armanisem
Photography or during transportation to and from any photo or video session
location, whether foreseen or unforeseen, however caused and whether or not
caused by negligence.

II. PHOTOGRAPHY RELEASE

(1) In consideration of my engagement as a model, and for other valuable
consideration herein acknowledged as received, I hereby grant AR.K Armanisem
Photography it’s professional photography assistants, representatives, and assign
the absolute right and permission to copyright and use, re-use, publish, and
republish at any time photographic portraits or pictures of me or in which I may be
included, in whole picture or in partial, or composite or distorted in character or
form, without restriction as to changes or alterations, in conjunction with my own
or fictitious name, or reproductions thereof in color or otherwise made through any
media at their studios or elsewhere for art, advertising, trade, or any other
marketing/advertisement purposes whatsoever.

(2) I also consent to the use of any printed matter in conjunction therewith.
(3) I hereby waive any right that I may have to inspect or approve the finished
product, the advertising copy, or any printed matter that may be used in
connection therewith or the use to which it may be applied.

(4) To the fullest extent allowed by law I agree to RELEASE, HOLD HARMLESS
AND INDEMNIFY AR.K Armanisem Photography it’s professional photography
assistants, representatives and assigns, and all persons acting under its
permission or authority, from any liability by virtue of any publication or use as
previously described, even though it may subject me to ridicule, scandal,
reproach, scorn and indignity.

I am over the age of eighteen years of age/or my parent or legal guardian has
also read and signed this Release below my signature.
(5) I have read the above authorization, release and agreement prior to its
execution, am familiar with the contents thereof, and have every right to contract
in my own name in this matter.

• THE TERMS OF THIS AGREEMENT SHALL ACT A RELEASE AND
ASSUMPTION OF RISK FOR MY HEIRS, EXECUTORS, ADMINISTRATORS,
AND ALL MEMBER OF MY FAMILY, INCLUDING ANY MINORS
ACCOMPANYING ME. I UNDERSTAND THIS IS A BINDING CONTRACT THAT
SUPERSEDES ANY OTHER AGREEMENTS OR REPRESENTATIONS AND IS
INTENDED TO PROVIDE A COMPREHENSIVE RELEASE OF LIABILITY BUT
IS NOT INTENDED TO ASSERT ANY DEFENCES WHICH ARE PROHIBITED
BY LAW. THE SPECIFIC RIGHTS OF THE PARTIES MAY VERY FROM STATE
TO STATE. IF ANY PART OF THIS RELEASE IS DEEMED UNENFORCEABLE,
ALL OTHER PARTS SHALL BE GIVEN FULL FORCE AND EFFECT.

Legal Rights

NRS 52.155 Trade inscriptions, signs, tags and labels. Inscriptions, signs,
tags or labels purporting to have been affixed in the course of business and
indicating ownership, control or origin are presumed to be authentic.

NRS 52.165 Acknowledged documents. Documents accompanied by a
certificate of acknowledgment of a notary public or officer authorized by law to
take acknowledgments are presumed to be authentic.

NRS 52.175 Subscribing witness’ testimony unnecessary. The testimony
of a subscribing witness is not necessary to authenticate a writing unless required
by the laws of the jurisdiction whose laws govern the validity of the writing.

CONTENTS OF WRITINGS, RECORDINGS AND PHOTOGRAPHS

NRS 52.195 “Duplicate” defined. “Duplicate” means a counterpart produced:
1. By the same impression as the original;
2. From the same matrix;
3. By means of photography, including enlargements and miniatures;
4. By mechanical or electronic rerecording, including a counterpart produced
by an optical imaging system;
5. By chemical reproduction; or
6. By other equivalent technique designed to ensure an accurate
reproduction of the original.

NRS 52.205 “Original” defined.
1. An “original” of a writing or recording is the writing or recording itself or
any counterpart intended to have the same effect by a person executing or
issuing it.
2. An “original” of a photograph includes the negative or any print therefrom.
3. If data are stored in a computer or similar device, any printout or other
output readable by sight, shown accurately to reflect the data, is an “original.”

NRS 52.215 “Photographs” defined. “Photographs” include still
photographs, X-rays and motion pictures.

NRS 52.235 Original required. To prove the content of a writing, recording or
photograph, the original writing, recording or photograph is required, except as
otherwise provided in this title.
